Aging Services provides essential social and health services to residents ages 60 and older who are functionally impaired. Through this program the County promotes social, physical, economic, and emotional well-being to encourage maximum independence and improve the quality of life for older adults, and their caregivers.
Case management is provided to actively enrolled participants and includes:
Identification of physical and emotional needs,
Development of a plan to address needed services,
Monitoring the quality and effectiveness of the services, and
Ongoing support to address changing needs.
